Caries are when teeth begin to decay and get soft. As decay gets deeper, it can lead to tooth aches and broken teeth. Many times getting a filling as a child can be a traumatic experience when the child needs to be numbed and then the cavity needs to be cleaned out and filled. For some children that may not be able to tolerate that procedure, other options are needed.
Silver Diamine Fluoride, or SDF, is a solution that is painted on to the decayed portion of a tooth. It kills the bacteria causing the cavity and actually can cause the soft portion to get hard again and “arrest” the caries. This is a good option for children or elderly patients that may not be able to tolerate the procedure of restoring a tooth in the traditional way.
One of the downsides is obviously the dark black staining. The black means that the SDF has worked, but it can be alarming to many parents and patients. Sometimes on posterior teeth this may be a more appropriate option and then anterior teeth may still need to be restored in the traditional sense. Other times it may be appropriate to apply this, especially to baby teeth with the understanding that it can stop the caries from progressing until the child loses the tooth naturally.

Why do we use this medicine?
SDF is a liquid antibiotic that can be applied to teeth with certain kinds of cavities or sensitivity. SDF kills the bacteria causing the cavity and strengthens that part of the tooth. It makes the cavity hard like concrete, so the cavity does not get bigger and the tooth becomes less sensitive. Using SDF “buys time” to fix the cavities – especially if your child is young or anxious and cannot cooperate for fillings or crowns.
